When Dr. Kay uses the closed technique, he makes incisions within the nose in order to sculpt the cartilage and bone. A closed technique can correct a crooked nose as well as tip and bridge irregularities. It can also reduce the size of the dorsal hump.

During the open technique, Dr. Kay uses internal incisions and an incision between the nostrils, which is the area called the columella. This technique allows him to easily see the bone and cartilage, making it ideal for symmetry corrections, such as nasal angles and tip deprojection.
